Did you catch the CDC's last installment of "Have You Heard? Facts From The Field - October 31, 2013"

This edition highlights Oklahoma State Department of Health’s (OSDOH) chronic disease Toolkit Trilogy which "brings together data and evidence-based prevention strategies to help Oklahoma communities make decisions to improve health outcomes." According to the issue, the toolkit can be used to "find out about cost savings from preventing hospitalizations, learn more about clinical and community prevention services, and view statewide data highlighting the connections between access to health care and chronic disease."
