Did you catch the CDC's last installment of "Have You Heard? Facts From The Field - October 31, 2013"
This edition highlights Oklahoma State Department of Health’s (OSDOH) chronic disease Toolkit Trilogy which "brings together data and evidence-based prevention strategies to help
Oklahoma communities make decisions to improve health outcomes." According to the issue, the toolkit can be used to "find out about cost savings from preventing hospitalizations, learn more about clinical and community prevention services, and view statewide data highlighting the connections between access to health care and chronic disease."

Have you seen the current issue of the CDC's OMHHE Health Equity Matters e-newsletter?
The Health Equity Matters e-newsletter is an electronic publication produced by the Centers for Disease Control and Prevention's (CDC)
Office of Minority Health and Health Equity (OMHHE) which intends to "promote awareness of minority
health and health equity issues" that affect the work at CDC and the
broader public health community.
